It depends where you are. In Boston, everyone calls methheads tweakers. But down south a tweaker can be anything. I know for a fact we call crack heads and coke heads tweakers

When someone does coke or meth and then starts peeking out windows thinking the cops are there, that's called tweaking out. Hence the term tweaker. That shit doesn't happen with someone on dope. A dope head just nods out.

Definitely something that's related to someone who's doing any kinda upper drug- meth/coke/crack.
